Invisible Hearing Aids: Lyric vs. Custom CIC vs. OTC: An Honest Comparison

Discretion is one of the most common things people ask about when they first start exploring hearing aids. Not everyone wants to advertise that they wear one. And the good news is that the options for truly discreet, or even invisible, hearing aids have never been better.

The less good news is that ‘invisible’ means very different things depending on which type of device you’re talking about. Lyric, custom completely-in-canal (CIC) hearing aids, and OTC in-ear options all promise some level of discretion, but they work differently, cost differently, fit different types of hearing loss, and suit different lifestyles.

This post compares all three honestly, including the tradeoffs most marketing materials leave out. By the end, you’ll have a clear picture of which invisible hearing aid option, if any, is the right fit for you.

Phonak Lyric 4: The World’s Only 100% Invisible Extended-Wear Hearing Aid

Lyric is genuinely in a category of its own. No other hearing aid on the market offers what Lyric offers: a device placed so deep in the ear canal that it is truly invisible, not just discreet, but invisible, and worn continuously for weeks or months at a time without removal.

How Lyric Works

A specially trained audiologist or otolaryngologist places the Lyric device approximately 4mm from the eardrum, deeper than any other hearing aid on the market. Once placed, you wear it continuously: sleeping, showering, exercising, and swimming in some cases. The device stays in place for approximately 6 to 12 weeks, at which point the battery depletes and you return to your provider for a replacement device.

Lyric uses analog sound processing, an important distinction from modern digital hearing aids. Sound is captured close to the eardrum, which Phonak argues produces a more natural listening experience because it takes advantage of the outer ear’s natural amplifying shape (the pinna effect). Many users find the sound quality very natural, particularly for their own voice. That said, analog processing means Lyric does not offer the programmability or environment-specific tuning of digital devices.

What’s New: Lyric 4 and Self-Replacement

The current model is the Phonak Lyric 4, which brought meaningful improvements in comfort (a 0.3mm reduction in device size led to significantly higher comfort ratings) and reliability (improved debris collection to reduce earwax-related failures). In May 2024, the FDA also cleared a self-replacement option for experienced Lyric users, meaning some patients can now replace their own devices rather than visiting the provider every 6–12 weeks, halving the number of required appointments.

Who Is Lyric Right For?

• People who want absolute invisibility and are willing to pay a premium for it
• People who don’t want to think about their hearing aids at all, no daily insertion, no charging, no maintenance
• Mild to moderate hearing loss only, Lyric is not suitable for more significant hearing loss
• People whose ear canals and ear health pass the candidacy assessment, earwax levels, canal shape, and overall ear health all affect suitability
• People who don’t need Bluetooth streaming, app control, or digital programmability
• People who can commit to the subscription model and regular provider visits

Lyric is NOT suitable for: people with severe or profound hearing loss, a history of ear infections, middle ear disease, skin disorders of the ear canal, or uncontrolled diabetes. A professional assessment is required to determine candidacy. Lyric is a prescription device, not available over the counter.

Custom Completely-in-Canal (CIC) Hearing Aids: Prescription Invisible With More Flexibility

Custom CIC hearing aids are the traditional ‘invisible’ prescription option. Like Lyric, they sit inside the ear canal. Unlike Lyric, they are removed daily, use digital processing, and can be programmed to your exact audiogram across frequencies.

They are custom-molded to your individual ear canal shape, which means the fit is precise and the device is stable. Most CIC hearing aids are nearly invisible, not quite as deep as Lyric, so a very thin pull cord may be visible at the opening of the ear canal, but most people around you won’t notice them.

OTC In-Canal Hearing Aids: The Accessible Entry to Invisible Hearing

The OTC category now includes genuinely discreet options, and one of the most significant developments is the ELEHEAR Delight, a completely-in-canal (CIC) style OTC hearing aid launched at CES 2026. This is a meaningful step for the OTC market, which has been dominated by behind-the-ear and earbud styles.

OTC in-canal hearing aids are designed for adults 18 and older with mild to moderate hearing loss only. They are not suitable for more significant hearing loss. Unlike Lyric and custom CIC, they are self-fit rather than professionally fitted.

Other OTC options offer varying degrees of discretion:

The key limitation for all OTC in-canal options: without professional programming to your specific audiogram, the customization ceiling is lower than either Lyric or custom CIC. They work well for many people with mild to moderate hearing loss, but not for everyone.

A Note on Earwax: More Important Than Most People Realize

All three invisible hearing aid styles are more affected by earwax than behind-the-ear or receiver-in-canal devices, simply because they sit inside the ear canal where earwax is produced. For Lyric specifically, excessive earwax production can affect device function and longevity, and is one of the candidacy factors evaluated before fitting.

For custom CIC and OTC in-canal options, regular cleaning and wax guard replacement are important maintenance habits. If you have known issues with excessive earwax, discuss this with your hearing care professional before choosing any in-canal style device, as it affects your experience more than almost any other factor.
